Technique one ...
1) Choose the two colors you want, one for the background and one for the actual scan-lines.
2) Filter > Render > Clouds.
3) Filter > Render > Difference Clouds. Press CTRL+F until satisfied, the more balanced it is between the two colors the better. And make sure it's not inverted.
4) Filer > Pixelate > Mosaic. Play around with the size of the pixels, the bigger they are, the bigger the squares of the Grid will be.
5) Filer > Stylize > Find Edges.
6) CTRL+I ... (invert)

Technique Two
1) Open a new document and make the size 5px by 5px. Take a the 1px pencil and make a line exactly down the middle, and one exactly across the middle. It should be a plus sign.
2) Look in Edit or File or one of those and go to Define Pattern and save it.
3) Open your original document that you want scan lines on, make a new layer above whatever it needs to be above, and then Edit > Fill > Pattern > (the pattern you just made).
4) Play around with the Opacity as you see fit. Feel free to press CTRL+I because White Scan lines sometimes are better looking than black ones, depending on the usage.
5) This step is optional. Take the Circular Selection tool, set the feather to 10 pixels or play around with it, and select random ovals of the grid and press delete. It should look like it's fading in and out at different places. Sometimes setting the feather higher like at 20 pixels, and then selecting and deleting the scan lines around any text looks cool.
